The Follow Program

Tree of Life's Follow Program is designed to serve CKD patients at any stage, pre-and post Kidney Transplant. The program will help increase patient compliance, non compliance as we now it, can results in disease progression, ultimately lead to possible death.

The program will address and reduce anxiety and depression, assess family culture, traditions and dynamics.

The Follow Program team works closely with the Therapists seeing patients at least once a week, and with the Treatment Plan Coaches (TPC) seeing patient twice a week.

Working with personal care team, family and other professionals to manage conditions such as diabetes, hypertension and other health conditions.

Our therapists will travel to private residence, skilled-and assisted living. In addition, Treatment plan coaches also includes travel to dialysis Centers.
